Do you believe that home is where the heart (and the best care) is? We are looking for a high-energy, empathetic Health Coach to join our team and support our Structured Family Caregiving program. In this role, you aren’t just a coach; you’re a champion for independence, dignity, and family-centered care. You’ll help patients stay in their own homes by supporting the incredible caregivers who look after them every day. The Mission You will be the bridge between our waiver members, their caregivers, and the clinical team. Whether you’re visiting a home to ensure safety or reviewing daily notes to spot where a caregiver needs extra support, you are the "eyes and ears" that ensure our clients thrive. What You’ll Do Be the Field Expert: Visit homes to perform initial assessments and ensure a safe, cozy environment for our members. Coach & Connect: Build deep rapport with caregivers—guiding, educating, and supporting them through virtual and in-person check-ins. Problem-Solve Like a Pro: Partner with RNs and Case Managers to escalate concerns and find solutions for evolving health needs. Stay Organized: Keep everything running smoothly by tracking your magic in Salesforce. Travel & Flex: Work mostly from the comfort of your home, with periodic trips to our Connecticut offices to sync with the squad. Who You Are An Empathy Expert: You have high EQ and a genuine passion for helping others. A Communication Rockstar: You can talk to anyone and explain complex things simply. A Tech-Savvy Multitasker: You’re comfortable navigating software (Salesforce fans, step to the front!) and love the fast-paced energy of a startup.
